today is my obs/gyn long case assessment day
posted to obs/gyn wards for 9 weeks, and i m happy that i got gyn for long case
dun really like (or hate!) obs!

very happy, when its my turn, Prof saw me n he actually brought me away to another case, which he think is better...so i like phewh~ good..i going to present to Prof, the senior obs/gyn consultant in SGH...someone who really appreciate ur knowledge n really good! so with all the good n high hope..i introduce myself to the patient nicely n start my 1 hour clerking.

my patient is a vr nice lady, 27 yrs old malaysian iban. G3P2, current in 23 weeks gestation.
was referred from KK Gita (Matang) cuz no fetal heart detected on daptone.
at SGH...scan was done n was told to have 'kandungan anggur' - a molar pregnancy.
ok good, time to shine! since i nvr expect to get this kind of case..but still good!

i think i clerk well with this case (associations, risk factors, past obs, etc) and had come out a vr good management plan for her..and yes...i even do a dipstix bedside..n protein was traced (?PE) so with all these in mind..i m ready to present this case to prof!!!!

SUDDENLY!!!!! I WAS BROUGHT TO THE OTHER LECTURER...THE WAT-THE-CRAP-SMKB!!!

and that time i really see SNOWSTORM!!!!!

i presented at least till management n follow up..but along the way i got irritated by so many atypical questions, which i would like to share with all of u!!! tamade!!!!

A is me; B is SMKB

1.
A: patient's blood group is AB, which is a risk factor for Molar preg
B: what!? then what is the paternal blood group? isnt that important!??
A: ...ok, sorry i didnt ask.....(so far i only come across the importance of maternal blood group, but also not proven yet!!)

2.
A: patient did not complaint of any abd mass or any acute pain, which i m worried of any theca-lutein cyst which tort and ruptured easily
B: ok..wadever, so do u noe how many % of patient will have theca-lutein cyst
A: in molar is around 10-30% (most literature quoted around 20%)
B: ur range is too wide, ok wadever u like
A: what!!???...

3.
A: uterus on palpation correspond to 20 weeks, not larger than date
B: so?? u should tell me the consistency!!!
A: (so so so so wad la! molar is a cause of uterus larger than date wad!!!) ok..it does not feel doughy!
B: ok, not dough, wadever u like

4.
A: so management is suction n curretage with oxytocin after procedure
B: what is the name of the thing they use to do SnC
A: er...its a curette with suction power..2 holes at the tips to prevent blockage
B: the name!
A: er......(thinking what special name tat thing have!!?)
B: SUCTION CURETTE!!!
A: mai high..that's not a name!!!

B: ok, tell me advantage of suction compared to normal currettage
A: ok..to prevent embolisation during procedure, as the tissue is sucked out..not further curretted into the uterus
B: ok wadever u like
A: ....

A: so after SnC, i will send the sample for HPE to confirm molar n to see any presence of invasion
B: tell me the characteristic HPE findings
A: ok, can see generalised/focal chorionic villi swelling + diffuse trophoblastic hyperplasia and +/- of fetal tissue
B: no, not specific enough!
A: ......
B: faster, i m waiting
A: ....sorry i duno
B: faster! i m waiting
A:......i really duno
B: hydroscopic degeneration of the cells.
A: ok. (wadever u back!)

A: during follow up i would advise for effective contraception, better to be barrier method, avoid IUCD and hormonal
B: why?
A: effective contraception, so when patient bleeds..pregnancy would not be an option; and since IUCD n hormonal can have breakthru bleeding..the presentation will affect my judgement on the progression of ds
B: ok, whatever u like!
A: ........

A: ok..so rare le..vaginal mets
B: tell me the exact site of mets in the vagina
A: er..........fornix?

其实很多时候我都在思考一个问题
一般人都觉得读医的应该都是很聪明很勤劳的那种人
还记得第一年进医科
我看到了很多聪明的朋友
我也看到很多很努力的朋友
我因为先天资力不足，所以只能靠后天的努力
终于在过去的四年里成绩还算不错

刚记得大家刚认识时
每个人都很有冲劲
很自动自发，连走路的速度都很快
很多事情也不会去介意做得太多

四年过去了
我所了解的情况也不再看得到了

很高兴的，
有一些朋友还是带着一样的态度
自动自发，很努力的在学习
但是，这群人毕竟少之又少
所以当表现的勤劳一点的时候
反而会遭来闲话

"XXX还会在哪里？一定是在ward啦! hahahaah!"

其实我觉得自己懒就算了
人家勤劳就请你把嘴闭上
不要为你自己的错误找一个借口

为什么我写这些东西？
因为我不希望在不知不觉中被同化

everything here is so different from my country
not only their thing, their ppl, their culture, everything,
is something i really impressed and enjoy living with it
i will tell u some vr impressive things,

their dogs are adorable..and they can jus keep their dog in their pocket
all their dogs look so tame and really vr dog
and somemore u seldom hear them bark
they make sure they are clean, and u can c dogs whenever u go
not the dirty dogs...clean n high class dogs

their food is delicious!

搞活动的，鼓励成立小group
大家开始讨论别人
鸡蛋挑骨头，每天出theory
我们讲的对，你们讲的错！
很不好

朋友们，
把你的社交圈子放大
不要极限每天都只面对一般读医的朋友
每天在这个圈子钻钻钻
好事都变坏事

我非好人
我的宗旨就是
不能忍受自己看不顺眼的东西，
就像我不可能逼你去吃大便，所以你也不要那你的冷屁股来贴我的脸

这几天都在下雨刮大风
家里这盆仙人掌也有好几年了
因为很有耐力，越长越高，
所谓树大招风，终于在几个月前历尽不起
到了下来
也许不可以说他经历不起大风
应该说那个花盆已经承受不了他的大了

所以老爸老妈就把他换到一个更大的盆
给他能够越长越大
不过在昨天，他终于又倒下了

老爸回来后
我告诉他，不如把他剪短吧
那样他就不会那么锋芒毕露了
老爸立刻告诉我
“剪掉！！？”
倒了不要紧！我们买更大的盆给他
再不够大，我们就把他种到地上！

老爸这种宁可死也不怕冲的精神让我很敬佩
很多时候，宁可死的精精采采
也不可以委屈求全
我是在这样的家庭里面被教育出来的

第四年的大学，
受了很多挫折
有想过不然就算了，

jus finish watched this japanese drama
iz called Iryu, like other medical drama
portray doctors in a un-realistic way
which may influence more ppl to into medic
but real life is not like drama, at all.

one nice thing about japanese drama is that
they put in a lot of life values and is more knowledge-orientated
if compared to western one
where they will stress on which doc sleep with wic doc
who love who, who got affair with who
which is actually vr irritating sometimes

iz a very interesting surgery-based drama
for medicine, the best will still be House!
jus that the cases in this drama is quite dramatic,
the scenario of the final surgery is:

9 month old, boys, known case of dilatation of left ventricle, is to undergone
Batista operation (removal of un-contractile portion of the ventricular wall).
Besides, iz a known case of situs inversus and there is only one coronary artery,
where the right coronary artery branched out directly from left coronary artery
rather than from the aortic sinus. Moreover, intra-op, they found a coronary
artery aneurysm on the left decending artery, which is undetectable during angiography
as it arise extraluminal, and the lumen remain intact. The aneurysm most likely
arised from previous Kawasaki Disease (common in kids).

the head surgeon immediately perform a by pass surgery with left gastro-omental artery
because the internal thoracic artery is still not well formed in kids. and followed
by the batista procedure.

really hope can go OSAKA next yr

我们这班人都很怪的
其实我觉得那些感去找医生诊断的医学生
是没有什么大问题的，至少他们能够接受自己有这种需要
我觉得一个人最重要的就是"Insight“
是你能够看到你自己的问题，然后接受它，然后改变它
很多人（特别强调的是我每天接触的医学生）
常常是我们讲的 ego-syntonic, 她们从不觉得自己有错
因为自己不懂，所以就不会去改，这种才是恐怖的一群

自从过了精神科，很想去找心理治疗师谈谈
我相信或多或少大家都有这种需要

医学生精神问题严重
我不赞同，因为我觉得应该是
医学生精神问题非常严重

i should be in serian now for the busy n hectic district hospital posting,
i reached serian this sunday, after unpacking,
found out that the hostel hav no electricity
after asking the sec guard and unimas staff there,
they said they have "bill tercicir" and will try to settle the next day (morning)
ok fair enough, since iz a shop house, and iz vr vr dark and damn hot
so no choice, have to drive back to kch again after unpacking

monday,
i drove back to serian again, early morning,
since we have briefing by posting coordinator and orientation to the hospital
after tat, found out that the electricity still didnt come back yet
so i cannot tahan alrdy, so i called to faculty administration office

they tell me they hav to tell dean
dean tell have to tell main campus at kota samarahan
kota said have to ask finance department to settle bill
after tat hav to wait sesco to go to the hostel

and yes,
iz tuesday alrdy, and the hostel is still black out
and i still hav to travel up n down everyday
and i cant study anything
and i cant do anything other than feeling extremely annoyed
with those slow n lousy malaysia-style admin ppl

stupid UNIMAS
cant u just settle the pathetic sesco bill earlier?

其实只剩一个礼拜就考试了，
但是现在外面下着雨，所以想写一点东西
先把读不完的书放一边

最近跟很多老同学谈起各自的状况
真的有很多朋友在医学系，都是从中学开始的朋友，
一些在英国，一些在澳洲，一些在莫斯哥，一些在乌可兰,
一些不知道在哪里，还有一些就留在古晋慢慢蹲

在不同的地方读医，有各自的利与弊
所以也开始看开留在本地留医也不一定是一个坏事
因为学医他不象其他科系，它注重的是人与人的交流，
和如何融入你服务的社群里
不管在哪里，大家好好拼吧，
你们都是很优秀的

一直都觉得其实还好
直到今年被分配到一个自己本身很不喜欢的小组
心情每天都收到影响而很难专心
其实我也了解不是每个人都和的来
就算和的来的好朋友，也有一天会因为意见不合
人嘛。

其实我真的是很显这组一部分的人
我觉得医学生都是怪人
我不排除我也是其中之一

k750i officially dead, after 2 weeks suffering with the
primitive Nokia 8310, finally decided to get this SE K550i
not better than K750, but my intention is to buy a mid-range phone
then abuse it in 2 years time! and only consider the need of pda while working
i dun tink i need one now, i may need a Harisson's medical book than
a samsung omnia haha!

another white stuff after white ikea desl,
macbook, sonic gear, apple remote,and apple mighty mouse

ok ok la...a sony ericsson 2 MP cybershot phone,
but the camera really not so good, compared to k750i
had taken more than 10k of photos with my k750i,
some of the photos some more published in newspaper, really good.

had gone back to swimming, although i m still fat n slow
and the stupid stampark entrance fee increase again,
this make this sport damn expensive,
but i tink this is the few sport wic i enjoy doing it,
not like the stupid jogging wic i hate to do it,
n since no one here to play futsal,
yeah, go swim like a fish (or frog) lo haha

swimming oso one of the best cardio-pulmonary workout
build up ur lung function and heart s well,
iz important for a guy like me who suffering allergic rhinitis
and with a strong family history of cardiovascular ds

even bought this goggles, quite expensive but worth
TABAKA View racing goggles, orange color
anyone wan to join me swimming?

this is a leachate pond; i feel leachate pond is like our community, a mixture of bad attitudes and behaviours, but do you see the grass near the pond? there's always hope.

a lot of seniors told me that, to see one's real personality, the best time is during clinical year,
in Sibu, where ur close friends can become the one u hate most. i dun really agree, for juniors who reading this blog, let me tell u, the time u will c one's real personalities is during community posting, cuz when ppl do things together, share their thoughts, and have too much time for u to think, problems arised. however, there's chance to discover new excellent values in friends, which i experienced that as well.

being competitive and being kiasu is total a different things to me. i admit tat i m a very competitive guy (which ppl may find this annoying), but i m a believer that competition makes one move forward, iz a positive competition where u look forward and works toward a target. the satisfaction from hard work is the best feeling in this world, where u feel u really deserve the success.

being kiasu is another thing, if u rajin and do all the procedures and read all the books, i really pay full respect to u as some of my coursemates who are great really do so, n i really respect them n proud to study together with them. but no doubt, therez another group of ppl even rush, plan, and psycho ppl jus to get the best out of everyone. a success from kiasuism is a shame.

u see, the problem is after ppl who know me read this post, some will agree, some will totally disagree, but its ok, because i really cannot care bout everyone's feeling here. i understand my ability in doing stuff, give an example, i know with my power alone i cannot stop ppl killing shark, but i know i can start with not eating sharkfin (doug taught me this, but i still eat la, haha!); however, some may feel they have the ability to do so, but, can you? pls, dun overjudge urself.

ok, for those who had finish or is currently in psychiatry posting.
pls do not diagnosed me.
i read in frontline, from dr gan, during psy posting, stop diagnosing ur friends and urself.

我是打鼓的，虽然节奏感不好，技术不好，也没有真正的训练，
但是，我还是喜欢打鼓哈哈。其实我真的觉得很荣幸可以在长屋，
和一班伊班朋友一起玩gong. 一边喝米酒tuak, 一边玩gong, 一边跳ngajat,
就这样过了一晚。

this is the first day we arrive in Rumah Jawa, Selalang, in the Meradong Area of Sarawak. i'm so touched and proud that s a sarawakian, this is the first time i step into a rumah panjang (longhouse) and we are invited by their traditional dance and all the penduduk q up and shake hand wif us, iz really an honor. this little girl, i love her looks, she sat there after dance, and suddenly jus fall asleep like that, haha so cute.

the tuai rumah said during the speech "he take doctors as god, what they say and act will nvr be wrong and the will follow until they die", i feel the burden and the responsibilities in me, s a future doctor, and as a proud sarawakian

yesterday i came across a news, a boy in London was born 19 years ago with a short left leg (short femur bone), and after 19 years of surgical correction, today he had both leg of same length and finally he can run like normal kids alrdy. how the limb is lengthened? its tru a very powerful mechanical technology, called the "Ilizarov Method", i m vr amazed by this method when i first come across it last year during my orthopaedic posting, and its really a very impressive and useful tools. lets me explain it in a simple way

usually when a bone is fracture, doc will open up and put metal across the fractured fragment (internal fixation), but in ilizarov is actually a kind of external fixation where pin is inserted through the skin and penetrate deep into the bone fragment, and was hold outside a circular ring. besides used for stabilizing fracture, it can be used to correct deformity as well, such as limb shortening, in the case above.

imagine this in a vr simple way, the bone is short, we going to make it longer, so how? first, we artificially break the bone, then we fix one pin on top of one fragment on top, then the other pin on the fragment below. and everyday,the patient will tighten the screw at the circular frame, to pull those 2 fragments apart from each other...and slowly..few milimeters per month..after few years..it can be lengthened, such as in the case above.

and a very gilik thing is, because the pin is fixed into the bone from outside the skin, so whenever u drag the 2 fragments apart, u drag n injured the skin as well, make sense rite?
but few mm of tear in the skin is consider a minor injury la

basically the concept is like tat la, but therez more things to talk on this powerful method.

this is how it looks like in x-ray after u fix ilizarov on a fractured tibial bone.
so next time when u c a patient walking with an circular frame around his leg,
yes, it is the ilizarov.

today is labour day, means going back to kch in 2 days time
the purpose to write this post is to keep a memory for myself
too lame to write diary, jus to write something down,
go into archives, so tat next time i can read back

this is the place i go almost 5 times a week in this one year, Sibu hospital, we had 6 postings here, too bad, score a consistent B+ for all postings, no improvement. I'd like to write down the case i got for each posting, medicine:SLE with nephrotic and iatrogenic cushing, surgery:acute on chronic PUD; paediatrics:acute exacerbation bronchial asthma;obs & gyn:unstable lie; orthopaedics:trauma related cystic swelling at leg. after all these posting, i can say i love medicine, but i like ortho as well, other jus so so only la...

i will definitely missed my room, iz big, HOT! dusty, and messy. iz a room with poor equilibrium capability, when hot, very hot, when cold, very cold and too big for me to clean every week, the bed though queen size, iz really bad with terrible lousy spring, which poke my back everynite! no matter wad, i still will miss this room

next, will be housemates. this is the longest time i had stay out from home,at first tot cannot cope but actually can one la. the world most precious invention will be washing machine and internet i should say haha, cuz life without a washing machine n internet is really crazy! thanks housemates for all the love..whahhaha..and sorry for tonnes of emo episodes..but hey! everyone of u oso got emo episodes la ok? but really amazed by how this ppl can play dota 5 times a week! haha, but play du play, dun forget give me back my cable haha. all the best guy!

haha, i decided to name him "Lucky"; go back will try make a sticker and paste on him "Lucky : Lost & Found". Kembara, tho iz really not a good car, but really got feel to me alrdy, i feel iz like my dog haha. but shit, still a court exhibit, have to drive back sibu at 20th May for court case, which actually spoil my holiday.

Sibu is the heaven for pirated DVD, i will try sort out how many DVDs i had bought, i agak agak i had spend >RM300 on them, good quality one woh, DVD version haha..to buy them, i recommend two places, one is at taman muhibah, another one in Sing Kwang!thinking of posting out a list of movies tat i had bought

the saddest thing is the departure of my grandpa, he departed during the second week of my paeds posting. i cant make it to talk to him at the final second, but he leave very fast without much suffering, i guess. haighz, tell u dun smoke liao lo, dun wan listen aiks. but iz ok la ah kong, i knw u cant tahan here without ah mah. so enjoy ur life up there, dun worry for me or anyone la, i will be a doc and ask other old men dun smoke too much! we will all miss u. haha, i go back will burn some cigarettes for u la, dun worry. aiks, like got many things to tell u la :( why cant u wait a little while more?

finally got my car out from police station,

finally Inspector Bahar ask me to go court,
to angkat sumpah, so that i can take my car back,
iz under duno wic section of law, that, to take out my car,
i have to follow the following conditions
1. cannot change color of my car
2. cannot modify my car
3. cannot change name or sell the car
4. must jaga baik baik my car
5. oral jamin RM150000
since the court case not yet finish, so i m responsible to send in my car anytime,
as an exhibit for the court case.

the whole angkat sumpah thing is very interesting,
i was told to be quiet when go into the room
and must bow when c the commissioner of oath (COO),
it makes me feel like i even have to take a joss-stick, kneel down, and pray haha
ok nvm

then when go in, the COO is an indian,
with a photo of him shaking hand with sammy vellu, hanging on the wall
i cant tahan alrdy when i saw tat.

then a guy walk towards me n start talking to me in foochow

Save Kembara Update

updates on my kembara.

thanks to tze and her dad, who appointed lawyer to due with the fussy inspector,
today he finally phoned me n asked me to go court this friday to sumpah,
so tat the court can return my car to me
so we c how first la, dun give too much hope first

so today went police station again wif insurance adjuster,
to take photos of the car for claims, as later on this car will be tow
to the court on friday!

amazing-ly, without me bringing my key,
the police there can buka my car, they got a pair of key s well
how did they do it? i not so sure? maybe they take from those thieves?
according to them the thieves kiao out my lock,
use them as a mould to make a key, then jus go in my car n drive away!

n since the car have no much damage,
not "hua" to claim insurance, s NCB will lost

the thieves stoled my car for 5 days
n the car was kept in balai for omos a month d
who are the real thieves?

First, i really have to thank Polis Diraja Malaysia,
they found back my kembara in 5 days time.

sunday noon i received a call from Insp. Bahar,
telling me that a kembara was found at Bintangor,
and need me to send in my green card to check the engine number.

by tat night, seems like the news is spreading fast and they really doing their job,
a police car with siren go to 68 Pelita Heights, my kch house,
and inform my dad that the car was found.

and today,
my kembara had been tow back to sibu,
from bintagor...so i went to have a look on him..

when i rchd sibu police hq, i spot this kembara and have a strong feel iz my car but iz wrong car plate so i feel aiks, iz not him la..so i went into the office and find Insp. Bahar...(actually it is my car)

they "kiaw" open the lock and go in tru the passenger seat.

they stolen the spare tyre.

as well as my Pioneer CD Player and the tweeters.

and they are smart enough to change the car plate to WHL 9086,
so guys, new toto and 4D number!

they caught 5 suspects as well,
i will find a "cara" to go in to kick those suspects' face
fuck them!
but to wait for them to release the car is another issue,
how long will the procedure takes?
anyone know any faster cara?
anyway, happy enough to find back kembara.

you never know how much u love them,
until you lost them.

在沙巴过年，
其实今年没有过年，
不要紧，去了一趟沙巴，
很爽一下，假期完了，
回去诗巫就要开始生孩子了，
不是我生啦！

Manukan Island 离沙巴州大概十五分钟坐船，游泳的话要3个小时。那些黑黑一条条的东西，是鱼来得，海水清澈到想游泳池！我是还没到过热浪岛啦，但是我对这里已经很满足了。太清澈了！

Sapi Island 就在 Manukan 的附近，每次在照片上看到这样的照片觉得很神经，那里可能那个水可以清到你可以看到那船在海底的影子！这次我自己看到的！不要玩玩！

Kundasang 是在神山的山脉群里，原本从那里是可以看到神山的，但是因为雾的关系，什么屁都看不到！那里有很美的梯田，卖着一大堆新鲜的蔬菜！

Kundasang War Memorial。跳吧，跳吧，跳久会跳到英国。

Marlin Fish Statue 和古晋的大白猫可能是朋友，不要神经！

Kota Kinabalu Waterfront，当然，古晋的waterfront还是最美的。但是这里可以清清楚楚看到一整粒太阳慢慢的沉进大海里。。。

现在我在小儿科
没有想像中的容易，
也没有我想像中的那么有趣，

他们不会告诉你哪里痛，

每天到病房一半的时间是在和小孩玩
玩好睡，睡饱吃，吃饱又玩
我也很想回到当我还是小孩的时候，
无忧无虑。

这些方块是老弟帮我在马六甲一间他所谓的古董店买到的，为什么要用这些东西呢？哈！每天要叫小孩排，18个月的小孩要会把六个方块叠起来，24个月8个方块。。。背这些号码就够我死了。

many ppl told me
"so boring one, i die oso i wont listen jazz"

well, it depends i suppose
i jus like jazz, i love diana krall, pink martini, oscar peterson, etc.
the way jazz music present themself is just way too cool.

a guitar,a bass,and drums.
thats all u need.
the best thing bout jazz is the "spontaneous" feel of all the performers.

i believed most alternative bands today
are actually evolved from old times jazz :)

"PUTRAJAYA: Newly-qualified doctors will have to undergo two years' compulsory housemanship from next year.

Currently, they only have to do a year of housemanship.

Health Minister Datuk Seri Dr Chua Soi Lek, who announced the Cabinet decision yesterday, said the move was considered important to increase the knowledge, skills and experience of graduate medical officers.

A rotation system of four months in six departments will be implemented. With the newest inclusion being the accident and emergency department, the rest are medical, paediatric, general surgery, orthopaedic, obstetrics and gynaecology.

Dr Chua said the ministry would apply to the Government to place them under the U43 grade upon completing the two years' housemanship compared to the current U41. The U43 pay scale is RM500 more than U41.

He said the doctors would be able to pursue their specialist courses or Master’s after serving two years' compulsory service compared to the current situation whereby they can only apply to further their studies after three years.

About 1,200 local medical students graduate annually."

i like to watch documentary about second world war,
dunno why oso, i ever told my family tat i feel
i was once a soldier during world war 2,
taz y everytime watch the documentary,
feel very real..haha..even dream of it sometimes.

this is an aerial view of Hiroshima. n the bull-eye is the target
of bombing. iz a revenge from the allied forces to fight back,
after the pearl harbour attack, as wad the claimed.

the crew who drop the bomb, the plane is named "Enola Gay"
i wonder how r those crew going to face their life after dropping the bomb
however, they r soldier,
soldier is jus to accomplish mission rite?
but they are human as well, rite?

today i learn few lessons.

1. if i c a chronic smoker, present with hemoptysis, think of lung cancer. if somemore presented with shoulder n arm pain, most probably will be pancoast. n i go n think it is jus a simple pleurisy. clever rite?

2. in kuching u forget put parking coupon is RM5, in sibu, they charge u RM20!!!

3. and once u get fine because u forgot put coupon, everytime after tat u will be very cautious, n will remember to put. and jus when i go another place n park my car, i scartch the coupon finish, n i only realised it is 5.00pm.
